Sponsors

Vendor Information

1. Vendor forms are available below for each venue.
2. Complete and sign the Vendor Application and Vendor Rules/Agreement forms.
3. Return the forms via fax to 904-777-6945 or scan and return via email to tiffany@varietyent.net.
4. If your application is approved, you will be contacted by the Vendor Relationship Manager(s).
5. Please make sure your email address and phone numbers are listed correctly on the application.
6. Once notified of your approval, please complete and sign the Vendor Contract and Electrical Requirements form.
7. Return the forms via fax to 904-777-6945 or scan and return via email to tiffany@varietyent.net.
8. Submit the full payment along with the security deposit to reserve your space.  Spaces will not be reserved until full payment and all signed documents, including insurance riders, are received.
9. Make checks payable to “Variety Entertainment” and return to 3020-I Prosperity Church Road, Suite 272, Charlotte, NC  28269.  The Company reserves the right to charge $35 for returned checks.
10. Once all documents and payments are received, the Vendor Relationship Manager(s) will assign and notify you of your space number.
